The River Watch: Community Initiative Designed to Provide Peace of Mind for Residents

River Watch by Tri‑State Cares is a community wellness and safety program designed for seniors who live alone. The program provides daily check‑ins, emergency notifications, and access to trusted community resources—helping ensure that no senior is forgotten or left without support.

For Participants: Peace of Mind for You & Your Family

  • Regular Connection: Daily Text, Voice Calls, or Email

  • Safety Net: If we can’t reach you, we’ll contact your designated emergency contact.

  • Completely Free: Supported by Tri-State Cares and generous donors.

Feed Your Soul: Nourishing Seniors with Care

Assists in filling in the gap for home delivered meals waiting lists. Provides home delivered meals for a limited time (up to three months) while clients are on the waiting list for meals through WACOG.

Qualifications:

  • Must be on WACOG's home-delivered meals waiting list.

  • Must live alone without a caregiver to assist with meals.

  • Cannot drive.

  • Must show financial need.

  • Must be 65 years of age or older.

Safe Steps: Supporting Seniors with Safe Living

Assistance installing grab bars, ramps and other projects supporting senior safety.

Qualifications:

-          Must have equipment ready to be installed.

-          Must have a financial need.

-          Must be 65 years of age or older or 62 years with a chronic health condition with debility.

Care Share: Medical Equipment Lending for Seniors

Providing our Hummingbird (volunteer) support to clients to assist with errands, friendly visits, socialization, etc.  

Qualifications:

  • Must not be receiving hospice services.

  • Must be 65 years of age or older or 62 years with a chronic health condition with disability.

Senior Link: Connecting Seniors to Community Care

Educating seniors and families on resources available within the community and assistance in navigating these referrals.

Qualifications:

  • Must not be receiving hospice services.

  • Must be 65 years of age or older or 62 years with a chronic health condition with disability.

  • Must agree to an assessment.

Provides loaner medical equipment (as available). Educating on fall risks and being safe at home.

Qualifications:

  • Cannot drive.

  • Must live alone.

  • Must not be receiving hospice services.

  • Must be 65 years of age or older or 62 years with a chronic health condition with disability.

  • Must agree to an assessment.

-          Must live alone.

-          Cannot drive.

-          Must not be receiving hospice services

Hummingbird Gardens: Transitional Care Home

Non-staffed transitional care home for homeless seniors or seniors facing homelessness with a terminal diagnosis.

Qualifications:

  • Must be in the process or screened to qualify for Arizona Long Term Care.

  • Must agree to contribute 80% of their income.

  • Must not be receiving hospice services.

  • Must be 65 years of age or older or 62 years with a chronic health condition with disability.

  • Must agree to an assessment.

  • Must be on hospice care.

  • Must be homeless or facing homelessness.
