Tri-State Cares is a non-medical support program offered at no cost or obligation. We help senior citizens in need by providing future care planning, information on local resources, caregiver support, and referrals.

About Our Group

Support. Guidance. Care. Independence.

We provide personalized support to help you stay independent and safe by connecting you with local resources such as home delivered meals, transportation services, and caregiving agencies. Our team can assist with long-term care and advanced care planning, help you explore options to remain safely in your home or consider other placement choices, and guide you through discussing health concerns or recent diagnoses. We also offer emotional support during challenging times, provide referrals to caregiver support groups and programs, and, when available, offer loaner equipment and volunteers for visits or errands.

Feed Your Soul: Nourishing Seniors with Care

Assists in filling in the gap for home delivered meals waiting lists. Provides home delivered meals for a limited time (up to three months) while clients are on the waiting list for meals through WACOG.

Safe Steps: Supporting Seniors with Safe Living

Assistance installing grab bars, ramps and other projects supporting senior safety.

Care Share: Medical Equipment Lending for Seniors

Provides loaner medical equipment (as available). Educating on fall risks and being safe at home.

Senior Link: Connecting Seniors to Community Care

Educating seniors and families on resources available within the community and assistance in navigating these referrals.

Hearts and Wings: Bringing joy to Seniors, One Hummingbird at a Time

Providing our Hummingbird (volunteer) support to clients to assist with errands, friendly visits, socialization, etc.

Hummingbird Gardens: Transitional Care Home

Non-staffed transitional care home for homeless seniors or seniors facing homelessness with a terminal diagnosis.
